At St Joseph Medical Center, we are committed to addressing our communities’ needs and have aligned our processes to follow CMS Health Equity guidelines in order to ensure patients are getting the resources they need to continue their journey of recovery once they leave our facility. St Joseph Medical Center has active relationships with community resource centers and health centers to help address Social Determinants of Health (SDOH) needs and provide health education to the community.
