Morton Hospital’s Patient and Family Advisory Council (PFAC) is the voice of patients and family members in the community who are interested in coming together to provide our health care team with constructive advice and feedback to improve our patients’ experiences. They are dedicated to recognizing community needs and helping to develop new services that best meet the expectations of patients and their families.

We are committed to keeping your Protected Health Information (PHI) private, and we are required by law to respect your confidentiality. If you would like a copy of your medical records, please click here to electronically request them or you can submit by fax. Our offices are opened from 8:00 a.m. to 4:00 p.m., Monday through Friday if you have any questions.

Admission
Upon your admission to Morton Hospital, we will work to get you to your room and make you comfortable as quickly as possible. An identification bracelet with your name and medical record number will be placed on your wrist for the duration of your stay.
