We are committed to keeping your Protected Health Information (PHI) private, and we are required by law to respect your confidentiality. If you would like a copy of your medical records, please click here to electronically request them or you can submit by fax. Our offices are opened from 8:00 a.m. to 12:00 noon, Monday through Friday if you have any questions.

Discharge Planning
Every patient is assessed for discharge planning needs by a nurse case manager. The nurse case manager works closely with you and your family, your physician, primary nurse, and the rest of your health care team to develop a safe and therapeutic discharge plan. That discharge plan provides for any continued care needs you may have after you leave the hospital.

Admission
Upon your admission to Good Samaritan Medical Center, we will work to get you to your room and make you comfortable as quickly as possible. An identification bracelet with your name and medical record number will be placed on your wrist for the duration of your stay.

Preparing For Your Stay

Pre-admission Phone Interview

Prior to your admission to Good Samaritan Medical Center, you will receive a call from a patient registration representative who will gather information for your medical records and answer any general questions that you may have. If you have not heard from us two days before your admission, please call us at 508-427-2240

Visitor Policy

Good Samaritan Medical Center recognizes the important role that visitors play in the care of hospitalized patients. Good Samaritan Medical Center makes every attempt to allow visitors without restriction.  You as the patient, and or your designated support person have the right to accept or decline visitors. Visitors may include, but are not limited to, a spouse, a domestic partner (including same sex partner), family members, and friends.
